Alvin grew up in Guyana and moved to Brooklyn in 1993. He was excited to come to the “land of the free and the home of the brave.” After moving to New York, Alvin worked in construction, and then as a driver. His hobbies include music - he was a DJ in his home country - and fitting cars with stereo equipment.

Two years ago while driving, someone hit his car from behind. He was in surgery for 12 hours. Alvin’s spine did not heal completely. He became disabled and is no longer able to drive. In addition to health struggles, this caused financial challenges and Alvin had trouble paying the rent.

He was connected to JASA’s Health Home program for older adults with multiple chronic health conditions. A JASA social worker helped him apply for the Emergency Rental Assistance Program (ERAP). She also arranged for a home care aide to visit him and helped him with transportation.

JASA’s Health Home program is for individuals that have Medicaid and two or more chronic health conditions. When clients are enrolled, a care plan is created and staff work closely with the client to complete the plan, which evolves with the client’s needs.
