The Milagros III

Milagros is a JASA Care Transitions client who is originally from Puerto Rico and moved to America at the age of 14. She met her husband here and they have been married for 54 years. Milagros is a proud mother, grandmother, and great-grandmother. She finds joy in attending church, decorating her home, and cooking traditional Puerto Rican cuisine. Recently, Milagros was hospitalized due to an infection. With the assistance of JASA, Milagros made a safe transition back home after her hospital stay.

JASA’s Care Transitions specialist conducted a home visit to assess her needs. The specialist observed that Milagros lacked certain essential items and was struggling to manage the medications prescribed by her doctor. To address these issues and prevent her from being readmitted to the hospital, the JASA team devised a plan. Milagros was given an automated pill box that helps her keep track of her medication schedule. Additionally, JASA staff arranged for Milagros to receive a recliner to alleviate her arthritis pain, as her current beach chair was not sufficient.
